Senior Nurse Case Manager – Rare Disease

Accountabilities:: Provide clinical support and education to patients and healthcare providers regarding diseases, therapies, treatment management, and available support services. Deliver inbound and outbound telephone support, including patient follow-up, maintenance, adherence, and other case-management calls. Apply established case-management protocols covering clinical monitoring, data collection, patient follow-up, compliance, therapy interruptions or discontinuation, and adverse event reporting. Coordinate the documentation and activities required to secure prescription drug coverage through private, public, and federal funding programs. Work with insurers, physician offices, healthcare professionals, pharmacies, and agencies to identify reimbursement solutions, coordinate medication delivery, and resolve access-related issues. Maintain accurate patient and case information, manage databases, and complete ongoing file and data audits. Communicate clearly with managers, team members, and relevant stakeholders regarding patient cases and case-management activities. Attend patient meetings, conferences, and specialized therapy training while developing relationships with patient associations, advocacy organizations, and pharmaceutical partners. Collaborate with program colleagues to develop and implement disease-management initiatives, patient materials, compliance parameters, needs assessments, and protocols. Support the onboarding and development of new Nurse Case Managers through activities such as shadowing and call monitoring. Requirements: Fluency in both French and English is mandatory. Bachelor’s degree in nursing or a Registered Nurse diploma; an advanced degree or relevant practice-management experience is preferred. At least 5 years of nursing experience. Experience in healthcare and case management within a medical or hospital practice, insurance environment, or biopharmaceutical setting. Experience with third-party reimbursement, pharmaceutical sales, managed care, clinical support, or a related field is an asset. Ability to understand complex medical information and communicate it clearly and appropriately to patients and healthcare professionals. Strong patient focus, customer-service orientation, telephone etiquette, and written and verbal communication skills. Demonstrated ability to work independently, prioritize competing deadlines, multitask, and perform effectively under pressure. Strong negotiation, interpersonal, leadership, analytical, mathematical, business, and financial skills. Excellent attention to detail, organization, and problem-solving abilities.
