We started with prenatal care and have already built a clinical platform used by OB-GYNs, along with a connected care program for mothers. Our products are designed to bridge the gap between healthcare providers and patients through technology, data, and AI-driven workflows. Our vision is much larger than pregnancy. We are building a continuum care platform for women's reproductive health, spanning from preconception and fertility through pregnancy, postpartum care, and beyond. As an early-stage startup with real customers and real deployments, we're looking for engineers who enjoy building products from scratch, solving complex problems, and taking ownership beyond writing code. ### About the Role We are looking for a Senior Backend Engineer / Product Engineer who can take ownership of products end-to-end. This is not a role where you'll work on isolated tickets. You'll collaborate closely with founders, product, design, healthcare providers, and engineers to build, ship, and scale products used in real clinical environments. You'll wear multiple hats, make technical decisions, contribute to architecture, deploy services, work on integrations, and help shape the future of our AI and data platform. ### What You'll Do * Build and maintain scalable backend services using Node.js and TypeScript. * Design APIs, databases, workflows, and system architecture for new products and features. * Work across the stack when required to deliver product outcomes. * Build and deploy production-grade services on cloud infrastructure. * Design and optimize PostgreSQL databases and data pipelines. * Work on integrations with hospitals, clinics, laboratories, EMRs, and third-party healthcare systems. * Collaborate with product and design teams to translate requirements into production-ready solutions. * Contribute to AI-powered workflows, LLM integrations, automation systems, and ML-driven product initiatives. * Participate in architecture discussions and help define engineering best practices. * Lead projects from idea to production with minimal supervision. * Mentor junior engineers and contribute to building a strong engineering culture. * Work closely with customers and providers during onboarding, deployment, troubleshooting, and product adoption. ### What We're Looking For * 4+ years of experience building and shipping software products. * Strong experience with Node.js and TypeScript. * Strong understanding of backend architecture, APIs, databases, and distributed systems fundamentals. * Experience with PostgreSQL and database design. * Experience deploying and managing services in cloud environments. * Experience building products from scratch and owning them in production. * Comfortable working in fast-moving startup environments with ambiguity. * Ability to understand business requirements and translate them into technical solutions. * Strong debugging and problem-solving skills. * Excellent communication and ownership mindset. ### Nice to Have * Experience with NestJS. * Experience with React or React Native. * Experience with Docker, Kubernetes, CI/CD pipelines, and cloud infrastructure. * Experience building AI-powered products, LLM applications, RAG systems, or ML-driven workflows. * Experience integrating with external APIs and enterprise systems. * Experience leading small engineering teams or mentoring developers. * Exposure to healthcare, HIPAA, FHIR, EMR, or hospital integrations. ### Who Will Thrive Here * Engineers who enjoy building, not just maintaining. * People who can move between architecture discussions and implementation. * Those who are comfortable wearing multiple hats. * Builders who care about product outcomes as much as technical excellence. * Individuals who take ownership and don't wait for detailed instructions. * Engineers excited by the opportunity to create real-world impact through technology. ### Why Join ONI? * Work directly with founders and influence product direction. * Build products that solve meaningful healthcare problems. * Contribute to AI-first healthcare systems with real-world deployments. * High ownership and autonomy from day one. * Opportunity to help shape the engineering culture and technical roadmap. * Be part of a company building for a large and underserved global market.
