Accountabilities: Analyze and reanalyze medical records to ensure documentation is accurate, complete, and processed within established timelines. Review electronic medical records and resolve items in documentation-related work queues when missing information becomes available. Follow up with nursing and ancillary departments to obtain missing medical record documentation. Contact healthcare providers regarding missing documentation or dictation that prevents accounts from being coded and processed. Update unbilled account tracking spreadsheets with the status of outstanding documentation and provide updates to management. Review medical record documentation, identify charting deficiencies, and enter them into the appropriate deficiency management system. Assign documentation deficiencies to the relevant healthcare providers and monitor outstanding items through resolution. Actively manage electronic and manual work queues to support timely medical record analysis and chart completion. Follow established company policies, standards, and system workflows when adding, transferring, and assigning accounts to appropriate work queues. Identify recurring system issues, process inefficiencies, and workflow concerns, escalating facility-related problems to management. Report trends or practices that do not comply with facility or corporate policies and standards. Document workflows, including process changes, modifications, and improvements implemented to enhance operational efficiency. Demonstrate openness to innovation and emerging technologies, including AI, to identify better ways of working and reduce friction for patients, clients, and colleagues. Perform additional duties as assigned. Requirements: Strong attention to detail and the ability to review medical records accurately while meeting productivity and timeliness expectations. Good organizational and follow-up skills, with the ability to manage multiple work queues, documentation requests, and competing priorities. Effective written and verbal communication skills for coordinating with healthcare providers, clinical departments, and management. Analytical and problem-solving abilities, including the capacity to recognize documentation gaps, identify recurring issues, and escalate concerns appropriately. A proactive, inquisitive mindset and willingness to learn, embrace innovation, and explore AI-enabled tools and process improvements. Ability to follow established policies, workflows, and documentation standards while maintaining accurate records of work performed. Preferred: Knowledge of Centers for Medicare & Medicaid Services (CMS) requirements and The Joint Commission regulations. Preferred: Experience using electronic medical record (EMR) systems. Preferred: Healthcare revenue cycle experience, particularly in health information management within an acute care facility. Certification requirement: Certified Revenue Cycle Representative (CRCR) certification must be obtained within the first 9 months of employment. Location and work authorization: This is a remote position limited to individuals who reside and are legally authorized to work in the United States.
